Care Manager submits request for a Broker & FI
Authorization letter is generated
Request submitted for start up budget which requires approval & effective date
Individual/family begins working with a Broker and FI. PCP and budget created
DDRO hosts a SD-101 training that individual / family must attend prior to budget approval
Budget reviewed by DDRO
Budget processed by OPWDD central office
DDRO generates approval letter
Individual/family call Front Door
Front Door Facilitator Assigned
- Eligibility Process
- HCBS Waiver Enrollment
- Choice in Care Coordinator
- Info Session
