The coordination of an individual’s care is done through a Care Manager who oversees and coordinates access to all services and supports; A Care Manager supports the individual with I/DD and their family throughout the Self-Direction process and works collaboratively with the Support Broker and FI. This service replaces Medicaid Service Coordination (MSC).
Specific to Self-Direction, your Care Manager is with you from the beginning of your Self-Directed journey. Once you’ve attended the Self-Direction 101 information session, the Care Manager will submit your request to OPWDD (local DDRO) for authorization of a Support Broker and Fiscal Intermediary (FI). Your Care Manager will help you interview and identify which Support Broker and FI will best meet your specific needs. The Care Manager will be your constant guide as you continue to navigate and use Self-Directed services. Additionally, your Care Manager can help you seek out and facilitate important conversations with potential providers of non-traditional and traditional supports and services.
